Hawaii accident and health insurance exam content outline
Last updated
The Hawaii Accident and Health exam follows Pearson VUE's content outline effective January 1, 2026. [1] It is a separate exam from Life. Hawaii has no combined Life and Accident and Health exam. [1][2] Below is every section, what each is worth, and the Hawaii laws behind the state part, including two topics you will not see on most other exams: temporary disability insurance and prepaid health care.
How many questions are on the Hawaii health insurance exam?
| Part | Scored | Pretest | Time |

| Accident and Health general knowledge | 50 | 5 | 75 minutes |
| Accident and Health Hawaii specific | 35 | 5 | 45 minutes |
| Total | 85 | 10 | 120 minutes |
Question counts are from the outline and times from the handbook. Totals are added from those numbers. [1][2]
- Pretest questions are mixed in, not marked, and do not count toward your score. [2]
- Passing score: 70. The handbook explains this is a scaled score, not the number or percent of questions you got right. [2]
- Fee: $75, paid when you book. [2]

Hawaii law common to all lines (23 questions)
The same 23-question block appears on the Hawaii Life exam: the Insurance Commissioner's powers, examinations, hearings and penalties; insurer definitions; producer licensing under HRS 431:9A; unfair and deceptive practices (431:13); premium accounting, commission sharing, records, controlled business; and the guaranty association. [1] See our Hawaii life outline page for that block in full.
Hawaii law for accident and health only (12 questions)
The outline splits this into three groups. [1]
A. Marketing methods and practices
- Prepaid health care (HRS chapter 393). Hawaii's Prepaid Health Care Act. Every employer who pays a regular employee monthly wages of at least 86.67 times the minimum hourly wage must cover that employee under a qualifying prepaid group health care plan. [3] Coverage must start once the employee has worked for the employer four consecutive weeks, at the earliest time the plan allows. [3]
- Temporary disability insurance (HRS chapter 392). TDI pays for disability from accident, sickness, pregnancy, termination of pregnancy or organ donation that is not work related. [4] Key numbers from the statute: [4]
- No benefits for the first seven consecutive days of a disability.
- Benefits last no more than twenty-six weeks for any disability or in any benefit year.
- If the average weekly wage is $26 or more, the weekly benefit is fifty-eight percent of it, rounded up to the next dollar, subject to the maximum.
- To be eligible, a person needs at least fourteen weeks of work in the prior 52 weeks, each with twenty or more hours paid, and wages of at least $400.
B. Policy clauses and provisions
- Mandated benefits (431:10-212, 431:10-217.5, 431:10A-115 to 121 and 133, 431:10A-206 to 208). Example: policies with family coverage on an expense incurred basis must cover newborns from the moment of birth for medically diagnosed congenital defects and birth abnormalities. If a specific premium is required, the policy may require notice of the birth and the premium within thirty-one days to keep coverage going past that period. [5]
- Cancellation (431:10A-106(7)). Under the optional cancellation provision, the insurer gives written notice, with the cancellation effective not less than five days later. If the insurer cancels, earned premium is figured pro rata. If the insured cancels, the short-rate table is used. [6]
C. Long-term care (431:10H)
Basic standards, outline of coverage, policy definitions and suitability. Under the basic standards, no long-term care policy may: [7]
- be canceled or nonrenewed because of the insured's age or declining mental or physical health;
- start a new waiting period when existing coverage is converted or replaced within the same company, except for a benefit increase the insured chose; or
- cover skilled nursing care only, or give much more coverage for skilled care in a facility than for lower levels of care.
Before you book
- At a test center you can take up to two exams in one session. On OnVUE online, exams cannot be combined. [2]
- Retake waits: 24 hours at a test center; two weeks for the first online retest, then four weeks for each later online retest. [2]
- HRS 431:9A-105(d) says exam scores are valid for two years from the exam date. [8]
